What to expect at your appointment...

We will make an appointment with you for the therapist that meets your needs by schedule, specialty and payment type. If you are using health insurance you will need to pre-authorize with your insurance company prior to coming in for the provider you will see. You will have a brief paper to fill out at your first visit. Co-payment or self pay payment is due at the time of service. Kindly provide us with 24 hours of cancellation if you are unable to make it to your scheduled time. If you have billing questions, you may call the office and speak to our Care Manager on Monday - Friday from 9 am - noon, 727-254-9183.

Our Care Manager answers your questions when you phone during these hours. Other business hours, please leave a message on our confidential voice mail and we will phone you the same business day. Care Manager answers phones Monday through Friday, 9:00 am - Noon, 727-254-9183.
